The term ‘Social Determinants of Health’ refers to conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. RMHF believes that improving these conditions are likely to have the greatest impact on health and well-being. We are interested in funding not only access to healthcare, but also your organization’s response to these Social Determinants of Health.

RMHF awards grants to Western Slope organizations offering programs and services that improve conditions for marginalized, socially, or economically disadvantaged people and are aligned with one or more of our current priorities:
